Question:  Is there a form for Parental Leave? Answer:  There is not an application form for parental leave unless combining it with Family Medical Leave (FMLA). Arrangements for the leave time (two weeks of paid leave immediately following the birth or adoption of a child) are to be coordinated with the units.
